Comm. Member Info Form - REQUIRED

We need to have up-to-date information on record for all households in the community in case of emergencies. like water leaks.  Additionally, Indiana State law (IC 32-25-5-3-1) requires Homeowner Associations to maintain an accurate roster of all community members and their contact information.
